JoAnn Manson, MD, DrPH, Chief, Division of Preventive Medicine at Brigham and Women’s Hospital, discusses the need for dietary supplements. The main purpose of dietary supplements is to treat deficiencies of vitamins and minerals or to prevent such deficiencies. For people who are concerned that they may not have an adequate diet, it's very reasonable to take a multivitamin or to take relatively low doses of different nutrients. Megadose supplementation, very high dose supplementation of any vitamin, mineral, or other nutrient, is not recommended due to the health risks. Groups that will benefit from dietary supplementation include women of reproductive age and pregnant women. Folic acid is extremely important in preventing neural tube defects and other birth defects so these groups should be encouraged to take a supplement of a folic acid. Also, calcium and vitamin D supplementation may be helpful for bone health, especially in older individuals who are at higher risk of osteoporosis and osteoporotic fracture or people who have lactose intolerance and tend to get very low intake of calcium or vitamin D-fortified foods. There are also some individuals, especially older people, who may have deficiencies of vitamin B-12 and could benefit from supplementation. It’s important to select a high quality supplement. There are a couple of things that will indicate whether a supplement has a high level of quality control. One is looking for the GMP label which stands for Good Manufacturing Practices. The other is USP, which stands for United States Pharmacopeial Convention. These labels will indicate that the supplement has been certified in some way in terms of quality control, that it has gone through a certain audit in terms of the good manufacturing practices, and that it's been tested for contents and for quality control. It’s recommended you speak with your physician before taking any dietary supplement. The first one I am going to recommend is minerals. You want to potassium, magnesium, sodium and calcium those were electrolytes, and then trace minerals. Because minerals helps reduce insulin resistance and it puts you into fat burning a lot better. Plus, these minerals are alkaline which offset some of the uric acid buildup that comes from the ketogenic diet to lessen the chance to get a gout or the uric acid kidney stones. Number 2 vitamin B1, this is essential in the adaptation of sugar burning into fat burning, and it is also great to relieve stress. Vitamin B3 is really good for lowering cholesterol that is coming out of the fat cells or liver and it is also good to support the liver in general. Vitamin B5 is another key B vitamin that is essential in the adaptation to fat burning. If you are deficient in B5 and B1, you may notice more fatigue, keto flu and keto rash, any of those symptoms. Number 5, vitamin A which is also essential in dealing with insulin resistance and you would get vitamin A in cod liver oil, grass-fed cheese, grass-fed liver and egg yolks. No. 6 vitamin D and I recommend vitamin D3 version, this is really good for insulin resistance and improving that. It is also in cod liver oil, egg yolk. In fact if you consume cod liver oil, you get a 50/50 split of vitamin A and vitamin D so you could kill 2 birds at 1 stone. No. 7 choline which is essential of getting the fat off the liver. I recommend doing electrolyte powder for the minerals. I recommend nutritional yeast for B1, B3, B5 and it even has choline. Some candida patients I've seen complain of many years of bloating, bowel and skin problems, perhaps recurrent ear, nose or throat infections and having taken repeated anti-biotics. For some women it could be an endless cycle of thrush and vaginal irritation or jock itch for guys, for others it may be anxiety, sugar cravings and bad PMS. For others it may be no interest in sex, joint pain, depression, toenail fungus or cracked heels, an increasing allergy to many foods and plenty more. It all begins with one product recommended by a website, magazine, health-food shop, friend or a practitioner, and after a few years the products just on coming. You may be thinking: “I’ll try this one for my digestion” or “This one will make me sleep better”. And so the list goes on. You soon find that you can’t keep them on the kitchen bench top; they are starting to take too much room so you put them in a box or container. Have you ever worked out the daily cost to you? Have you noticed yet that all these pills aren’t making that much difference when it comes to getting rid of your yeast infection permanently? I believe that the more products a person takes, the more product focused they will eventually become and the more likely that they become “symptom prescribers”. While there isnt a pre-market review of dietary supplements, says Frankos: "All dietary supplements that are marketed must be safe." The onus is on the manufacturer to ensure that dietary supplements are safe, but if FDA does identify a problem, through the literature or through adverse event reports, then "we can take action against those supplements and we have done that...." So the way I look at a supplement is it's an insurance policy. Now everyone has car insurance but no one plans to crash their car. In terms of supplementation you have to figure out that if an athlete is working out more and maybe eating less food than they need because they have to decrease body fat. When you work out more your needs go up, because you're working out harder and you're trying to lose body fat, trying to build muscle, what have you. So as the needs go up you want to make sure you're getting all the nutrients your body needs without the extra calories. A lot of people feel that if they're taking a multi-vitamin or a supplement of any kind then they don't need to eat their fruits and vegetables, that they're good. And I disagree, there's a lot of things in the fruits and vegetables that we don't even know about yet. So food is always first. Supplements are there just to help out with any kind of deficiencies and in this country you don't have to worry about deficiencies. It's not really about living, it's about optimizing your performance and that's really what the supplements can help you do. "
